HUD Definition of Homelessness: person living in emergency shelter, hotel/motel paid for an agency or organization, and/or place not meant for human habitation. Examples of places not meant for human habitation: building with no electricity or no running water, vehicle, abandoned building, tent, chicken coop, park, etc.

HUD Chronic Homeless Definition: an individual must have 4 separate incidents of living in a place not meant for human habitation or emergency shelter for a cumulative of 12 months over the past three years, or one continuous year of homelessness.

PSH provides participant-focused, home-based case management. The case manager develops an individualized service plan with the participant that is designed to provide consistent, long-term supportive service case management to ensure that participants maintain their housing. The case manager will meet with the participant at least once a month to keep sure the unit is in proper order, the lease requirements are fulfilled, and that the participant is connected to all necessary community supportive resources. The participant will pay 30% of his/her income towards their housing expense.
