PSH provides participant-focused, home-based case management. The case manager develops an individualized service plan with the participant that is designed to provide consistent, long-term supportive service case management to ensure that participants maintain their housing. The case manager will meet with the participant at least once a month to keep sure the unit is in proper order, the lease requirements are fulfilled, and that the participant is connected to all necessary community supportive resources.
